
General Terms and Conditions
Applicability
These general terms and conditions apply to all treatment agreements and coaching trajectories entered into between Koert Psychosomatiek (hereinafter: "the practice") and the client (hereinafter: "you" or "the client").
By making an appointment, you agree to these terms and conditions.
In cases where these conditions are deviated from, this shall be done exclusively in writing and in consultation.
Appointments and cancellations

Appointments can be scheduled by phone, email, or via the booking system.

Appointments that are not cancelled at least 24 hours in advance will be charged.

In the event of force majeure or unforeseen circumstances, a suitable solution will be sought together.
Rates and payment

The current rates can be found on the practice's website.

Treatments fall under non-contracted care and must generally be paid for by yourself. An invoice will be sent to you by email afterwards.

For coaching programs through an employer, the business rate applies, excluding VAT, unless otherwise agreed.

The payment term is 14 days after the invoice date, unless otherwise agreed in writing.
Reimbursement and health insurer

You are responsible for submitting the invoice to your health insurer yourself, if applicable.

The practice is not responsible for the amount of the reimbursement or its processing by your insurer.
File creation and privacy

During your treatment, a file is maintained in accordance with legal requirements (WGBO and AVG).

Your data will be treated confidentially and shared with third parties (such as your general practitioner or occupational physician) only with your consent.

Liability

The practice provides care and guidance to the best of its ability, within the applicable professional standards.

The practice is not liable for damages resulting from the provision of incorrect or incomplete information by the client.

In any event, liability is limited to the amount charged for the relevant session.
Force majeur

In the event of force majeure (such as illness of the practitioner, severe weather, a pandemic, or technical malfunctions), the practice reserves the right to reschedule appointments or to be temporarily unable to provide services.
Complaints procedure

The practice is affiliated with a recognized complaints procedure under the Healthcare Quality, Complaints and Disputes Act (Wkkgz).

If you are dissatisfied with a treatment, you are kindly requested to discuss this with the practitioner first. Should this not lead to a solution, you may make use of the external complaints procedure, listed on the website.
Applicable law

These terms and conditions are governed exclusively by Dutch law.

Any disputes will be submitted to the competent court in the district where the practice is established.
Changes to terms and conditions

The practice reserves the right to amend these general terms and conditions at any time.
